Optimal integration of care pathway functionality within the digital front door

The Enovation patient portal brings together various e-health applications with the aim of offering a clear and trusted starting point for the patient; one place where notifications and tasks from digital applications converge.

The implementation of the eCarePaths module is a great example where optimal integration is ensured for both the patient and the healthcare provider. The module was developed through a collaboration between EHR supplier NEXUS, the Dutch Gelderse Vallei Hospital (for care pathway content knowledge), and Enovation Funatic. The contribution of Gelderse Vallei Hospital has resulted in 70 standardized care pathways.

Development program

With the participation of Groene Hart Hospital, a joint development project was started at the beginning of 2024 to make the eCarePaths module available on Enovation’s Cloud platform. This has led to a new user interface that guides the patient even better through the care process.

The Enovation Funatic patient portal gradually makes information components available in the portal or app, depending on where the patient is in the care process. All this is based on the automated control of the process. In this way, the patient is provided with information at the right time.

Making new information or an action available within the eCarePath leads to a notification via email, SMS, or push message to the patient. When the patient then logs in, a dashboard opens where an action center is displayed. This ensures that the patient immediately sees all pending actions. The eCarePaths module has different types of actions that can be taken from the action center, such as reading new information, filling out a questionnaire, or reading a new message. With this action center in combination with the notifications sent out, the patient is helped with what is expected of them.

Go-live of pre-operative screening care pathway

Last year, Groene Hart Hospital started digitizing the pre-operative screening. With the activation of the eCarePath ‘Preparing for your admission’, 66% of patients no longer need to go to the hospital for pre-operative screening. The information request takes place digitally, after which the anesthesiologist assesses the data and can directly transfer it to the EHR status management. This means an improvement for both patients and healthcare workers.

Next steps

In the coming period, more and more eCarePaths will be launched by Groene Hart Hospital. The Pain Center has recently been made available, and the rollout of another 6 eCarePaths is planned for this year.
