- Nurses in the outpatient clinic apply electronically for aftercare;
- The Transfer Point complements and ensures that all information is sent to the care providers and the indication counter (Specialist in Elderly Medicine, CIZ or the municipality);
- The specialist in geriatric medicine, the CIZ or the municipality reports what is indicated;
- The healthcare provider informs when the patient can be taken over;
- The nurse makes the transfer that the care provider can view.

Every day thousands of Dutch people are referred by their GP to the hospital or other healthcare institution. Enovation POINT helps to properly organize all the information required for this transfer. This leads to fewer costs and better care. What Enovation POINT offers:
- Referrals from the general practitioner via a Regional Coordination Point to a VVT institution;
- The Regional Coordination Point has capacity insight into regional and national VVT institutions, so that the search is shortened;
- Automatic message to the doctor concerned. As a result, the GP knows when a patient is leaving the hospital, and especially to which follow-up care and with which care;
- The digital discharge message to the general practitioner enters the General Practitioner Information System (HIS) without additional actions.
Around 4,000 Dutch GPs now use Enovation POINT.
Enovation POINT is made available to GPs via the VIPLive portal of Calculus. The GP uses the existing and trusted VIPLive software and through an intelligent integration the data ends up in Enovation POINT. The regional coordination point and the VVT institution then work via the trusted platform POINT.
It is important that the correct information from the hospital, nursing home or doctor reaches the home care organization. Directly with the specific home care team, or via a central reporting point that forwards the file. Fast, safe and without hassle. With Enovation POINT, chain partners can digitally create, update and view all transfer projects. This prevents administrative red tape. Moreover, no specific hardware or software is required.
